Step 1: Account Verification

To register, you must have received your first SuperEOB statement. During the registration process, you'll be prompted for a Statement Number, located in the upper right-hand corner. In the form below, please enter the requested identification information. The information you enter must match your HPS member record. If you need assistance, call Customer Care.

You must enter a Statement Number from a SuperEOB sent to you within the last year.
You can find your group number on your health insurance ID card. If you do not have an ID card, please contact our Customer Care Team.
